We are interested to know about your personal experience as a person with a disability accessing health care so that we can improve services to better meet your needs.
Please tell us about your experience with the health system. You can talk about any aspect you like both negative and positive and in whatever setting you like, community health, hospital, GP.
All questions are optional. Your confidentiality is assured as we are not collecting any personal information.
The information will be used for planning and quality improvement. It will also be shared with service providers at forums, conferences and other public events.
