Welcome!

In October 2011, Governor Dayton approved a statewide safety initiative (MnSAFE) to reduce the frequency of workplace injuries to state agency employees.

As part of this initiative all state agencies are required to submit goals and objectives to the Governor's Office on how we will reduce the number of injuries within our agencies. MnDOT’s top two causes of injury are slips, trips, and falls and overexertion.

In an effort to reduce these injuries, MnDOT is seeking input from all employees on how we can prevent future injuries caused by slip, trips, and falls and overexertion. Employees can provide input through the following survey as well as participate in a brainstorming conversation led by supervisors.

MnDOT needs your input to know what will work best to make your work place safer. Your responses to this survey are anonymous, and will be summarized and reported out with those of the other survey respondents.

This survey will take just a few minutes to complete. Please complete the survey by Friday, November 2nd.

Please be specific in your responses. For example, if you think that “more time” or “more staff” are needed to make your workplace safer, please describe why – Time to do what? Staff to do what? If you think that different or additional equipment is needed, please describe what it would be used for.

Thank you very much for your time and participation. If you have any questions about this project, please contact Todd Haglin at todd.haglin@state.mn.us or 651-366-3079.
