District Middle School Survey 2015

The families and staff members of the District Local Control Accountability Plan (LCAP) Survey Sub-committee created this survey to learn what you think about both, your and your child's/children's experiences, in the Berkeley USD schools. By participating in this survey, you are providing important feedback regarding funded programs and actions designed to meet district goals. This survey is anonymous and demographic information is asked to assure that all of the Berkeley community is represented. The survey will take about 15 minutes to complete.
