In 2004, California voters approved Proposition 63, which created the Mental Health Services Act. One component of this act, Community Services and Supports or CSS, pays for mental health programs and services that improve access to underserved populations, bring recovery approaches to current systems, and provide “whatever it takes” services to those mental health consumers most in need.

Tulare County's Department of Mental Health is requesting the community's help in assessing Tulare County community needs. Your participation in the Stakeholder process would be appreciated. It will take a approximately 10-15 minutes to complete the following questionnaire. If you have any questions or would like printed copies of the questionnaire, please contact Tulare County Department of Mental Health at (559) 624-7458 or MHSA@tularehhsa.org.

If you are completing this survey on hardcopy, you can mail your hardcopy to: Tulare County MHSA, Attn: MHSA Community Planning Program Assessment Survey Coordinator, 5957 South Mooney Boulevard, Visalia, CA 93277.
