Dear Patients,
Ontario Shores Centre for Mental Health Sciences (Ontario Shores) wants you to have access to your health record (patient chart). To do that, we will provide you (and with your permission) your family access to the Ontario Shores online patient portal.
The internet-based portal will let you look at your own health information, cancel and view appointments, view your current medications and access discharge instructions and educational materials.
You will be able to access your own health information any time you want. This means you will get your information faster.
The portal is secure, private and confidential. Only you and anyone you give permission will be able to use the portal.
All that it will take for you to access the portal will be a computer with internet access and an email address for setup.
We expect the portal to be available in December 2014.
To help us make sure the portal meets your needs, the Project Team would like to hear from you. We have 15 questions we'd like you to answer to help us understand your needs. It should take less than five minutes to complete our survey.
