This survey is being conducted by United Cerebral Palsy of Los Angeles, Ventura & Santa Barbara Counties (UCPLA) for the purpose of developing materials and services to assist families of children with developmental disabilities transition from high school to adulthood. Creation of this survey evolved from multiple discussions with families navigating transition, school district professionals and Regional Center staff engaged in transition activities. Your input is extremely important in shaping the content, format and dissemination of transition training tools to be developed by these agencies.
Your participation and responses will be kept completely confidential. In appreciation for your completing the survey you will have the opportunity to enter to win four (4) passes to Universal Studios - Hollywood. Only one entry per family.
If you have more that one child with special needs between the ages of 15 and 22, and would like to complete an additional survey for each child, please send an e-mail to asimons@ucpla.org to request additional survey access.
