Thank you for participating in this survey. Your responses will help inform the Kansas Special Health Care Needs (SHCN) Program in our strategic planning process.
The following pages outline five priorities identified through previous strategic planning processes. These include:
1. Care Coordination
2. Direct Health Care Services
3. Behavioral and Mental Health Services
4. Training and Education
5. Family Caregiver Health
For each section, if you believe we have missed something, there will be an opportunity for you to add in additional services for consideration. Please make sure you are ranking those based on the question as well. This will assist us in prioritizing appropriately and making the most informed decisions.
