The following is a statement of informed consent. It restricts the use of the data collected to the named project by the named investigators.
By proceeding to the survey you are acknowledging that:
You have read the information regarding the nature of the survey and have been given the opportunity to discuss the information and your involvement in the project with the researcher/s.
The procedures required for the project and the time involved have been explained to you, and any questions you have about the project have been answered to your satisfaction.
You understand that your involvement is confidential and that the information gained during the study may be published but no information about you will be used in any way that reveals your identity.
You understand that you can withdraw from the study at any time, without affecting your relationship with the researcher/s now or in the future.
