introduction

Welcome to the Leadership Action Checklist 360 questionnaire.

There are two elements to the questionnaire:


1. The observers questionnaire which is filled in by the people you have requested feedback from.

If you haven't already sent us your list of email addresses of those people please do so as soon as possible so that we can get invitations out to them. Criteria and guidance were contained in the original email to you.


2. The self assessment questionnaire that you fill in about yourself.

It will take about 20-25 minutes to complete. We recommend that you allow the time to complete the questionnaire in one sitting.

Please think carefully about your responses, but if you find you are deliberating too much, then bear in mind your gut response is often the most accurate.

The "comments" elements from observers will be extremely valuable, but you may also find it useful to record the context, and some detail for your own answers .

Remember, we’re asking about the actions and the behaviours that you actually use.

This process is designed to help you focus on the areas you need to develop and the more honest your responses, the more useful the information it will generate.

Feedback is provided anonymously and will not be attributable to any individual feedback provider, unless they are in a feedback provider group of one person, e.g. line manager.

You will receive a written report with your scores and comments from all feedback providers. We do encourage you to have conversations about your feedback but only if your feedback provider explicitly agrees this with you.
