* 1. (1) By proceeding with this survey, I agree to allow Research Unlimited to use the information I provide to identify and match me to ongoing research studies. Our complete privacy policy is available on our website at researchunlimitedllc.com.  I agree that I have read the privacy policy in full.

(2) I attest that I am 18 years of age or older. This data will be screened for age requirement, individuals under the age of 18 years of age are ineligible to participate and their information will be deleted immediately upon completion of this survey. 

(3) I understand and agree to allow Research Unlimited will contact me (based on my preferred method of contact) with information about current and future studies that I may be eligible.

(4) I understand that I can skip/no provide a response to the questions in this survey that I do not feel comfortable answering.  You do not have to answer any questions you do not feel comfortable answering. 

(5) I understand and acknowledge that completion of this survey will not guarantee me access to research studies or monetary incentives or rewards.  I understand that Research Unlimited will share and provide my information with organizations conducting research studies in order to register as a user and to participate in studies. I will only be contacted by Research Unlimited if it is deemed that I am eligible for and/or meet the study criteria outlined by the researchers.

(6) I understand that Research unlimited does not guarantee acceptance into research studies. Compensation for participation in research studies will be provided by the Researcher/Organization conducting the project not by Research Unlimited.

(7) I understand that my information will be kept on file until such time that I withdraw this request.  If I choose to have my name removed from this list I can do so at any time via email at info@researchunlimited.com or by calling our corporate office at (804) 788-8901.
