Please complete this survey after finishing your classroom artist residency. Here you may enter the responses you, the classroom teacher, and the artist agreed upon at the reflection meeting. As you and the artist have seen, the questions ask you and the artist to make joint global assessment of the cognitive skills, social competencies, and personal development of the students in your classroom. The purpose for having the teacher and artist, jointly, complete this assessment is to ensure both reach a consensus with respect to the cognitive skills, social competencies and personal development of the classroom students that were part of the artist residency.
Do not complete this survey until you and the artist have had a conversation about the residency and came to a consensus on responses to this assessment of your classroom.
