2013 CTO Evaluation

Thank you for participating in the 2013 CTO evaluation. Board and TSC members may complete the CTO evaluation survey through November 30, 2013.

This evaluation is based on the CTO's performance during the current year. The CTO has provided a self assesment, which should be reviewed and referred to as you complete this evaluation.

The evaluation survey organizes the objectives under high level categories that appear in the blue heading. The objectives appear as numbered questions under the heading along with the agreed upon deliverable.

You must supply comments for those objectives to which you give the CTO a score of 5 or less. Scores of 5 or less submitted without comments will not be included in the overall average scores.

Board and TSC members must supply their names and contact information at the end of the survey. Submissions that do not match a valid Board or TSC member name will not be included in the overall average scores.
