Thank you for your interest in participating in an online focus group regarding the need for training and technical assistance to build, expand, and sustain transportation programs at health centers and community-based organizations.

Data collected from these focus groups will be utilized without any identifiable information of participants. Before registering for the focus group, please read the following consent statement, which describes your rights as a study participant.

Question Title

* 1. I understand the purpose of this study is to learn about the training and technical assistance needs around starting, expanding, and sustaining transportation programs at health centers and community-based organizations. I understand that HOP, as part of its research process, is conducting an online focus group with representatives from health centers and other community-based organizations.

I understand that my name will never be used on any materials (written or otherwise). No personal identifying information, including the name of my employer, will be published. I understand that participating or refusing to participate in this study will not affect my employment or services available to me from HOP and that I can refuse to continue in the study at any time.

My participation in the focus group is completely voluntary, and the only compensation I will receive is a $25.00 Amazon.com giftcard and an official acknowledgment in the final project's report. There are no known risks for participating in this study; I understand there is no penalty if I choose not to participate. The results of this online survey will be used for transportation-related research purposes only. I understand that I can learn about the study’s results in a report that will be available in summer 2014.
