Thank you for participating in our anniversary give-away. When you complete this short survey, you will be entered in all 5 drawings, as well as the Grand Prize Drawing. You only need to do the survey once - your name will stay in the pool.

Our drawings will be random, but we do want to make sure our promotion creates lasting benefit for local food, so we have some basic eligibility requirements:
1. Operational food hubs with sales
2. To accept offer, winners will need to sign our standard 2 yr license agreement and pay other licensing and subscription fees not included in promotion.
3. License agreement term will be from August 2014 – July 2016.
