2015 Community Health Assessment Youth Survey

Please answer the following questions about your health and the health of Iredell County.  Your responses are completely anonymous.  There will be no attempt to identify those who take this survey. 

 

The purpose of this survey is to gain knowledge on what the residents of Iredell County are facing in regards to their health.  The Iredell County Health Department is conducting this survey.  Your responses will be compiled together with other responses throughout the county and will be used to evaluate the health needs and concerns or Iredell County residents.  The results from the survey will be made available in written and electronic form.  It should take around 10-15 minutes to complete the survey.  If you do not live in Iredell County you do not need to answer any questions.
