ICC and ICC Patient Navigation Committee Introduction

The Indiana Cancer Consortium (ICC) is a statewide network of public and private partnerships whose mission is to reduce the burden of cancer in Indiana. This statewide network includes over 100 diverse partner organizations in specialty areas including health care providers and delivery systems, cancer programs, health insurers, public health agencies, professional organizations, minority groups, health profession schools, advocacy groups, research institutions, and wellness organizations.

The ICC Patient Navigation Committee is one of eight action teams and committees dedicated to implementing and evaluating the Indiana Cancer Control Plan 2010-2014. It is the mission of this committee to connect, educate, and share best practices among patient navigators in Indiana, so that together we can successfully reduce barriers and increase access in services specifically related to cancer. Thus, with the following assessment, the ICC Patient Navigation Committee aims to begin building a substantial network of patient navigators and patient navigator programs fostering collaboration and education across the state.

Please take the next 10 to 15 minutes and help the ICC Patient Navigation Committee better connect the work of patient navigators across Indiana in addition to identifying those barriers challenging navigators and their patients on a daily basis. The following assessment is to be completed by all patient navigators and their employers. Please send questions to Caleb Levell at Caleb@indianacancer.org.
