The IHEBA is a way primary care providers systematically assess for health risks and gaps in knowledge about prevention activities. These assessments are customized to the age of the patient, and should be part of Initial Health Assessments (for new patients), and as part of regular health maintenance visits for all PHC patients. There are a variety of ways each office, clinic, group may choose to implement this. In order for PHC to assist you in implementing the SHA, it is very important for us to know the approach you will undertake in implementing this state requirement.
