Introduction

As a gastroenterologist caring for patients with IBD, we would like to invite you to participate in a short survey. The survey is part of a programme called IBD Ahead 2014 led by a steering committee of global IBD experts.

The survey’s primary objective is to learn more about what outcomes you believe are important to measure in Crohn’s disease (CD) and ulcerative colitis (UC) patients and how you assess the long-term prognosis for patients diagnosed with IBD. We would like to know specifically which clinical, demographic, laboratory and phenotypic factors you consider to most influence a patient’s risk of progressing to a more complicated disease course. We also want to know how important you think certain patient outcomes are for IBD patients.

Your input will help the steering committee to understand current practice and identify gaps in clinical care to determine key questions requiring guidance around the use of predictors of CD outcome in clinical management.

This survey is comprised of 3 sections and should take approximately 15 minutes to complete. Please use the ‘next’ buttons at the bottom of the page to advance to the next section of the questionnaire. All questions are mandatory and you will be unable to move to the next page until all questions have been answered.Your responses to the survey will remain anonymous if you do not provide contact details.

Please complete the survey by 18 July 2014.

Thank you very much for taking the time to complete the survey; your participation is highly valued.
