Informed Consent

You are invited to participate in a research study if you are an adult (18 years or older) and identify as a sexual minority (lesbian, gay, bisexual, queer). The purpose of this study is to explore the influence of religion and spirituality on sexual minorities.

If you choose to participate, you will be asked to complete a survey about your religion and spirituality, psychological well-being, attitudes toward your sexual orientation, symptoms of depression, and basic demographic information about yourself. The survey will take approximately 30 minutes to complete. It is completely anonymous. Participation in this study is voluntary. You may withdraw from study participation at anytime without any penalty.

Participating in this study poses minimal risk. You will be asked to disclose your sexual identity. The items within the questionnaires may induce self-reflection, which continues after taking the survey. Should this lead to any discomfort or concern, please contact the investigator or dissertation chair, Dr. Robert Switzer, by email (provided below).

You will not directly benefit from this study. However, I hope the information learned from this study may benefit society in our understanding of how religion and spirituality influences sexual minorities.

All information regarding your participation will be kept confidential. Your name will not be collected in order to secure the anonymity of your responses. All survey answers will be stored electronically on this researcher's external hard drive and will be kept in a secure, locked location in the researcher's home. This researcher and the dissertation committee will be the only people who have access to the data. After five years, the data will be deleted by this researcher (per the American Psychological Association guidelines).

At the end of the study, you will be given the option to provide your email address in order to have the possibility of receiving compensation for your participation. Submitting your e-mail address is completely optional. If you do submit your email address, you will be entered into a raffle to have the chance to win one of two $25 gift certificates for Amazon.com. The raffle drawing will be done after the data is entirely collected. If you win, the gift certificate will be sent to your e-mail address. If you chose to provide your email address, it will not be stored with or connected to your survey answers.

If you have any questions or concerns regarding this study, please feel free to contact the investigator, Kevin Hiner, at kch4817@ego.thechicagoschool.edu or dissertation chair, Dr. Robert Switzer, at rswitzer@thechicagoschool.edu.
