Initial Survey or providing information for each additional dog
The survey consists of two parts, ownership information and individual dog information. If you own only one dog, complete both parts.
For each additional dog, skip the ownership information and complete a survey for each dog that you own.
If you co-own any Silk Dogs, please coordinate with the other co-owner(s) so that only one entry is made for the Individual Dog information section of the survey.
For deceased Silk dogs, report only those alive between 2007 until the present.
