Entry Form - Halloween Office Decorating Contest

 
All entry forms must be submitted by October 21st.

Judging will occur on Friday, October 25th. The winning office will receive a catered continental breakfast.

Winners will be announced at the Faculty/Staff Halloween Monster Mash event on October 30th. Off-site locations are encouraged to participate and submit photos of their décor by October 25th.
