If you provide ongoing care or assistance to a family member or friend you may also be known as that person's carer*. If the person you care for is in or has recently attended a hospital we would like your feedback. This will allow us to continuously improve our services. In the interests of improving carer recognition and engagement in hospitals, any issues you tell us about may be shared however any information which may identify you/the person you care for will be removed. The survey should take approximately 10 minutes to complete.
*A carer is a person who (without being paid) provides ongoing care or assistance to someone who has disability a chronic health condition, mental ill health, or who is frail.
