Thank you for your interest in the Specialist Palliative Care and Advance Care Planning Advisory Services Project survey.

The survey is being conducted by a consortium of national health, academic and aged care organisations, including Palliative Care Australia, Respecting Patient Choices, CareSearch, the University of Queensland, Queensland University of Technology, the Australian and New Zealand Society of Palliative Medicine, Leading Age Services Australia and Aged and Community Services Australia.

It is part of a multi-faceted project funded by the Australian Government, which includes a dedicated national hotline for GPs and aged care providers to call for immediate access to specialist palliative care advice and information about advance care planning. The service will be free and available 24 hours a day, 365 days of the year, and complemented by a suite of web-based resources, with tablet and smart phone access, as well as training and linkage activities.

The survey should take about 15 minutes to complete and the findings will help to ensure the advisory service resources are developed to meet the needs of your local community.

To participate,please complete the Informed Consent Form that follows before you proceed to the survey questionnaire.

Please complete the questions from your personal viewpoint, not with regards to the entire practice you work in. Other doctors in the practice will also be asked to complete the survey. It is recommended that the survey is completed in one sitting so answers are not lost.
