Guided Transition Interest

The Guided Transition Program is open to youth (age 14-20) and their family members. There are some participation requirements:
- Taking initial baseline survey
- Participating in 1 Person Centered Thinking training and 1 other training
- Meeting with staff person to review transition plan/graduation program
- Attendance for at least 1 tour
- Complete follow up survey
