All programs are asked to complete the Multiple Program Track Survey prior to their reaccreditation. This survey is meant to identify differences between program tracks if multiple program tracks or modalities are a feature of the program. The completed survey shall be due to CAHME six months prior to the start of the program’s self study year on February 1st.

Accredited programs seeking to add an online track or modality must also complete this survey along with their notice of change.

Completed surveys will be reviewed at the Spring Candidacy Committee Meeting for final disposition. A report of decisions will be sent to the Accreditation Council.
