Introduction

NAMI would like to hear from you about your experience as a parent/guardian of a student living with a mental health condition. The college years are often an important time in the lives of many young adults, particularly for those with mental health needs. We want to ensure there are services and supports available to help all young adults succeed academically and socially in college.

This survey should only be completed by parents/guardians of individuals diagnosed with a mental health condition who are currently or were enrolled in college within the past five years. Unfortunately, we are not collecting information at this time from parents/guardians of students who attend online colleges.

NAMI will use the information gathered in this survey to develop a set of standards and resources that colleges should make available to students living with mental health conditions and their families.

We estimate that the survey will take up to 20 minutes to complete. The survey will close on November 31st, 2011.

We greatly respect the confidentiality of the information shared as part of this survey. The results of this survey will only be reported in the aggregate. We will not share any of the individual responses or individual information provided in the survey without your explicit permission. We will not disclose individual identification information that is gathered as part of this survey.
