The Mayor's Alliance on Substance Abuse (859-225-3296 x. 651) is conducting an anonymous survey to increase our understanding of prescription drug issues in Fayette County. Parents or guardians of middle and high school students are being asked to complete this survey.

Your answers are confidential and will never be linked to your name, email address or other personal information. Information from this survey will help us design effective prevention programs.
