Dear Participant,

This letter is a request for you to take part in a research project designed to learn more commitment to Alcoholics Anonymous. This research study is being conducted by Principal Investigator Megan R. Dillow Ph.D., and co-investigators Shannon T. Carton, PhD Student; Virginia Nardi, MA Student; Timothy Gozanski, MA Student, and Nicole Johnson, MA student. This study is being conducted for professional development and to understand why people commit to Alcoholics Anonymous (AA). The results will be aggregated and may be submitted for presentation at the 2013 annual meeting of the National Communication Association conference.

You must be 18 years or older and a member of Alcoholics Anonymous to participate in this study.

Please complete the following questionnaire by describing messages that people have used that encourages your participation in AA. Then please complete the following scales and basic demographic information. Read each question carefully, and be careful to answer all components of the question if you can. If you are unable to answer a question, please leave it blank. There are no right or wrong answers to any of these questions. You may also choose to leave any question blank without any penalty.

Do not place any marks of identification anywhere on this questionnaire. Your involvement in this study will be kept anonymous. Participation in this study is voluntary. Your job status, your grades, your class standing, and your membership on an athletic team cannot be affected by either your refusal to participate or withdrawal from participation. You can stop at any point without fear of penalty. There are no risks associated with participation in this study. Completing and returning this questionnaire indicates that you have agreed to participate in this study. This questionnaire takes about 30 minutes to complete.
