Trans is an umbrella term referring to the spectrum of gender identity and expression. 

The information on this survey is designed to assist trans persons in choosing health or mental health providers. The questions relate to the providers’ knowledge and practice of trans-related medical and mental health care, as well as characteristics of an office environment where trans persons would feel welcomed.

Survey responses will be available online in the respondents’ exact words. Consumers will be able to post feedback on services they have received through a process that is monitored by a system administrator. Once you have completed the survey please email a high resolution headshot or logo that you would like to be used for your profile on the website to hcrate@bouldercounty.org.
