EXercise for Type One Diabetes (EXTOD) EDUCATION

We are conducting a multi-centre UK study called EXercise for Type One Diabetes (or EXTOD) EDUCATION. We are gathering information about current practices, both nationally and internationally, regarding education of patients with Type 1 diabetes, specifically on how to manage exercise with diabetes.

One of the key things we would like to collect is the type of resources you provide to patients or information that you signpost patients to.

Information we collect will be used to generate a set of guidelines to support health care professionals support patients with Type 1 diabetes. All centres that contribute would be able to access this resource. We would like to get your views and experience.

The survey should take approximately 15 minutes to complete.

All data and answers you provide will remain confidential and where possible, anonymous, unless you agree otherwise. The possibility to forego anonymity will be assumed where you agree to share resources (which for instance may contain your employer logo/name), although all answers will be analysed and reported anonymously. If you provide your contact details then you agree to be contacted for the purposes stated throughout this questionnaire.
