Instructions:
1. Please assess the applicant based on the totality of your experience with them, including your experience scheduling the interview and other insights gleaned from body language, attitude, etc.
2. If you feel you do not have enough information to adequately rate the applicant on a particular element, please select the “Unable to Rate” response.
3. While the report is designed to minimize the need for extensive narrative, you are welcome to add as many comments as you would like to give staff more context. Also, please feel free to comment on aspects not included in the report, but that you feel are relevant.
4. We ask that the report be completed no later than 1 week from the interview, but we recommend that you complete the report the same day as the interview so that your thoughts are fresh.
5. Once you complete the online report, please email Meghan Maloney (mmaloney@escsc.org) to let her know.
6. Staff may want to further discuss the interview and your impressions, so please keep your notes (you are not asked to submit them along with the report).
7. Staff will let you know the decision about accepting the applicant into the consultant corps and whether the applicant accepted.
