Question Title

1. Please tell us about your experience with the Environmental Health Services Division (EHSD) by choosing one of the following areas/services (all answers are 100% anonymous):

I applied for a Permit, Inspection or other service in person at your Rutherford Lane office.

An inspection or investigation was conducted on my Food Establishment or Event, Swimming Pool or Spa Facility, Custodial Care Facility, property, etc.

I reported an environmental health complaint to be investigated by EHSD; i.e, food, pools, animal enclosures, junk vehicles, trash & debris, tall grass & weeds, dangerous structures, stagnant water, etc.
