2012 USA Ultimate US Open Application
Thank you for your interest in attending the 2012 USA Ultimate US Open Championships. This on line form will us to process applications in a more efficient manner.
The US Open will be held in Colorado Springs, CO on July 4th - 8th.
Applications must be completed by Friday January 27th, 5pm Mountain Standard Time. Notifications will be emailed to invited teams (and alternates) by February 24th.
Teams receiving bids to the tourney must be approved by their national governing body. Due dates for team payment, rosters, and required paperwork will be communicated when bid notifications are sent out.
