The Interstitial Cystitis Association (ICA) is collecting information to improve its awareness campaign activities. The goal of this survey is to gather information about your experience with the webinar in which you have participated, titled, “Creating a Positive Patient-Provider Relationship.” This survey should not take more than 10 minutes to complete, and you can refuse to answer any question or end your participation in the survey at any time. Your answers to the survey questions will be kept strictly confidential throughout the entire project and your name will not be associated with your responses in any of the reports or documents resulting from this survey.
