2014 US Open International Application
Thank you for your interest in attending the 2014 USA Ultimate US Open Championships. This on line form will help us to process applications in a more efficient manner.
The US Open will be held July 3 - 6 in Blaine,Minnesota, United States.
Applications must be completed by Friday January 31st, 5pm Mountain Standard Time. Notifications will be emailed to invited teams by February 14th.
Teams receiving bids to the tourney must be approved by their national governing body. Due dates for team payment, rosters, and required paperwork will be communicated when bid notifications are sent out.
