DISTRICT STAFF NEEDS ASSESSMENT SURVEY

Dear District Staff:
The Individuals with Disabilities Education Improvement Act (2004) requires that a Needs Assessment Survey be conducted annually for the purpose of planning inservice opportunities that may affect outcomes for learners with disabilities in schools.
This survey will take less than 10 minutes; please take this time to answer each question regarding your needs.
