Are you a carer in Cornwall? The Cornwall Carers Service is there to support you in your caring role and would very much appreciate if you could spare a few minutes and share your opinion of the service. Your feedback will be used to help shape and improve the Cornwall Carers Service to ensure it meets your needs and the needs of carers across Cornwall.
Most of the questions require simple tick box answers and we have provided additional space for you to give us more information should you wish. While we have tried to provide as much freedom as possible to answer as many or as few questions as you like, in some places you will see an asterisk. These questions will require an answer to avoid you seeing questions that are not relevant to your experience.
We know your time is precious and, as a small thank you, we hold a prize draw every quarter where we randomly select a respondent to receive a £25 voucher that can be spent in a wide variety of shops.
