The survey has been approved by the Colorado Department of Education Educational Data Advisory Committee (EDAC). Your responses to this survey will remain completely confidential – no one besides you and the Colorado Creative Industries' research analyst will see your information. And, you will receive a customized report based on your responses which will provide comparisons to the aggregate responses from all Colorado public schools. That report will also remain confidential and will be given only to you.

Instructions: You may provide the information collected via this survey in one of two ways. The first is to complete the questions via this on-line survey. You may return to complete the survey as many times as necessary.
