CDPHE Data Stakeholder Survey

The following survey is designed to gather information from stakeholders on how CDPHE is meeting public and environmental health data needs including data access, format, products, and CDPHE website functionality for dissemination of data and data results. Your input will help CDPHE to improve future data dissemination products. Please note that data suppression, availability of sub-state-level data, and needs for specific data indicators are outside the scope of this survey. The primary objective of the survey is to gather information on your needs and preferences on how you access and use data from CDPHE.

Your responses will be anonymous, but you will be given the opportunity to provide your name and contact information if you would like us to follow up on or respond to any of your comments and recommendations.
