Introduction
As part of a county health assessment, the Broome County Health Department is conducting a survey to learn about the important health issues in our community. The purpose of this survey is to learn about your health needs, whether you think the health services that you use in Broome County meet your needs, and if you have any difficulties in getting to or using these health services.
Please answer the survey questions as best you can. No one will be able to identify who you are based on your responses. This is not a test and there are no right or wrong answers. You do not have to answer questions if you feel uncomfortable. The information that we learn from this survey will be used to help health planners.
