Thanks for your interest in improving patient experiences. I am developing a Patient Service eLearning program and accompanying book for hospitals. I want to use stories that are based on actual patient experiences. I am looking for terrific examples as well as terrible examples of patient service. Names will be changed so confidentiality is assured. Hospital names will not be used in the stories.

There are 5 open-ended questions for you to answer. If I select your story/example, I would like to send you an autographed copy of my book (as a token of my appreciation) so please make sure you include your contact information in the final box so that can be done.

Responses will need to be given for each question asked – if you don’t have a response, please put NA (for “no answer”) so that the survey system will allow you to submit your story. Thank you in advance for your help!
