The goal of this research project is to better understand the role of the geneticist in the patient-centered medical home. This study has been approved by the UAMS IRB (IRB# 203189). The survey responses will be anonymous and take about 10 - 15 minutes to complete.
At the end of the survey, you may provide a phone number or email address if you would be willing to be contacted for a brief phone interview. The contact information you provide will not be associated with your survey responses, will not be given out to any other sources, and will not be stored by myself or others at UAMS.
There will be no direct benefits to those participating in the study. Participants are not expected to experience any psychological, physical, financial, or social harm. No participant identifying information will be collected through the survey. Certain parties such as the UAMS IRB, other institutional oversight offices, and the Office for Human Research Protections (OHRP) may inspect the records.
