Consent Form

RESEARCH PROCEDURES
This research is being conducted to examine social support for autoimmune thyroiditis patients within the context of marital relationships as well as the communication strategies utilized to enable effective support. If you agree to participate, you will be asked to take a brief survey that will take approximately 30 minutes. At the end of the survey, you will be asked if you would like to participate in interview that will take approximately one hour.

RISKS
There are no foreseeable risks for participating in this research.

BENEFITS
There are no benefits to you as a participant other than to further research with potential to improve communication for other autoimmune thyroiditis patients.

CONFIDENTIALITY
The data in this study will be confidential. We will not ask for your name or any other identifying information except for contact purposes if you agree to participate in the interviews. A separate link will be provided at the end of the survey to provide contact information to ensure your survey answers remain anonymous. While it is understood that no computer transmission can be perfectly secure, reasonable efforts will be made to protect the confidentiality of your transmission. All interview data will use a code instead of your name and only the researcher will have access to the identification key. Each interview will be audiotaped and the data contained in the tape will be encrypted. The name of the interviewee will not be captured.

PARTICIPATION
Your participation is voluntary, and you may withdraw from the study at any time and for any reason. If you decide not to participate or if you withdraw from the study, there is no penalty or loss of benefits to which you are otherwise entitled. There are no costs to you or any other party. Individuals participating in the interview portion of the research study will be provided with a $10 Amazon gift card.
