You are being invited to participate in a survey on treatment of behavioral disturbances of dementia. This survey is being done by Jessica Goren, from the University of Rhode Island in collaboration with Ryan Carnahan of the University of Iowa, Joshana Goga of Sheppard Pratt and Dorothy Williams of Shawnee Mission Medical Center. You were selected to participate in this survey because of your work with academic medical centers and their affiliated hospitals.

The purpose of this survey is to better understand when certain medications are used in clinical practice for patients with dementia. If you agree to take part in this study, you will be asked to complete an online survey/questionnaire. This survey will ask about your practice setting and treatments used for patients with dementia. It will take you approximately 10 minutes to complete.

You may not directly benefit from this project; however, we hope that your participation in the survey may help improve treatment for patients with dementia.

We believe there are no known risks associated with this survey; however, as with any on-line related activity the risk of a breach is always possible. To the best of our ability your answers in this study will remain confidential. We will minimize any risks by creating a de-identified data set for analysis, keeping all files on a password protected computer in a password protected file. Once creation of a de-identified dataset is complete, all identifiable data will be destroyed.

Your participation in this survey is completely voluntary and you can withdraw at any time. You are free to skip any question that you choose.

This survey was deemed exempt from IRB approval by the Camrbidge Health Alliance IRB on February 24, 2014. Completion of the survey will serve as consent to participate.
