Welcome

20% of survey complete.
At Boston Children's Hospital, we are developing a curriculum on Disclosure and Apology after adverse medical events and errors.

We will design an interactive program to help you develop your skills in communicating with patients, families and colleagues about medical errors and adverse events.

We are interested in better understanding your needs as a practitioner in relation to this Disclosure and Apology curriculum. This survey should take approximately 5 minutes to complete.

Your participation in this survey is entirely voluntary. All information will be anonymous and any results that are reported by us will be de-identified and presented in aggregate form.

If you choose to fill out the survey, you may leave any question blank and you may end the survey at any time if you choose not to complete it.

If you have any questions about the survey or the Advancing Relational Learning Worldwide project, please contact the project Education Leader, Donna Luff, at 781-216-2538 or donna.luff@childrens.harvard.edu

If you have any questions about your rights as a research subject, any research-related concerns or complaints, or if you want to speak with someone other than the researchers, you can call the Office of Clinical Investigation at (617) 355-7052.

Thank you in advance for your time and interest.
