Aquarium on Wheels Survey

Thank you for taking a few moments to fill out this survey of your experience with the Aquarium On Wheels program (formerly known as Youth A.L.I.V.E.!) at the National Aquarium.

The purpose of this survey is to help us learn about you, and how the program is making a difference in your life today.

If you are a current Aquarium on Wheels student, we ask that you fill out this survey twice each year. Once at the start of the school year (October - December), and again in the summer (June or July). For Seniors or others leaving the program and not coming back next year, we have a special Recent Graduate Survey we would like you to complete.

 

If you are an AOW Alumni, we have a special section of the survey we would like you to complete and keep us up-to-date on what's happening with you.

Your honest feedback is important! You do not have to share your name with us, if you do not want to. All your answers will be kept confidential.
