Informed Consent Form


1. INTRODUCTION
You are invited to be a participant in an experiment looking at hickey giving behaviors among college students. We ask that you carefully read this document and ask any questions you may have before agreeing to participate in the study. The study is being conducted by Steven Phillips, a student of Dr. Renee Countryman.
2. BACKGROUND
The primary purpose of this study is to examine dominance, submissiveness, and insecurity as they relate to hickeys.
3. DURATION
The length of time you will be involved with this study is approximately 10 minutes.
4. PROCEDURES
Once you agree to participate in the current study we will ask that you fill out the survey to the best of your ability. The questions you will be asked will pertain to details about how, how often, and whom you give/receive hickeys to/from as well as personality traits relating to dominance, submissiveness, and insecurity.
5. RISKS/BENEFITS
This study has the following risks: The only risk of participating could come if you become uncomfortable answering questions about your experiences with hickeys. If you wish to not answer any question you can skip them or choose to withdraw.
The benefits of participation are: Your participation will allow researchers to better understand how and why hickeys are given.
6. CONFIDENTIALITY
The records in this study will be kept private. All data collected, will be stored in the office of Dr. Renee Countryman. Only the researcher, Steven Phillips, and Dr. Countryman will have access to the records. No questions in the survey will ask you to identify yourself.
7. VOLUNTARY NATURE OF THE STUDY
Your decision whether or not to participate will not affect your current or future relations with Austin College or any of its representatives. Your participation is completely voluntary, even if you begin the questionnaire, you can still decide to withdraw from the study at anytime without punishment or consequences.

The researcher conducting this study is Steven Phillips. You may ask any questions you have right now. If you would like to contact someone other than the individual conducting the survey or you would like to speak to a member of the Austin College faculty about this project, please contact Dr. Renee Countryman at rcountryman@austincollege.edu.


9. STATEMENT OF CONSENT
You will be given a copy of the previous form to keep for your records.

The procedures of this study have been explained to me and my questions have been addressed. The information that I provide is confidential and will be used for research purposes only. I understand that my participation is voluntary and that I may withdraw at any time without penalty. If I have any concerns about my experience in this study (e.g., that I was treated unfairly or felt unnecessarily threatened), I may contact the Chair of the Institutional Review Board or the Chair of the sponsoring department of this research regarding my concerns.
