Introduction: HQIP is seeking feedback from stakeholders on how well we support the re-invigoration of clinical audit as a whole, and how much we help your work on audit in particular.

Confidentiality: This survey has been anonymised - personal contact details are not being recorded, and this includes respondents' IP addresses. Responses are automatically limited to one response per computer and email sender to further ensure the integrity of results.

Guidance on completing the survey: This form is grouped into the following sections:
