Informed Consent

Informed Consent for Participating in this Study

You are invited to participate in the research project entitled “The Interpersonal Self of a Transplant Recipient and Adherence” conducted by Brett Crowell under the supervision of Dr. Nancy Dorr of the Psychology Department at the College of Saint Rose in Albany, NY. You must be 18 years old or older and have received an organ transplant to participate. 

You will be asked to fill out a questionnaire about your transplant regimen, adhering to that regimen, and your quality of life. You will also be asked to answer some questions about self-concept, and illness intrusiveness. This study will take approximately 20 minutes to complete. Upon completion and submission, participants will have the option to enter a raffle for a chance to win one of four $25.00 gift cards to Amazon.com

Participation in this study is voluntary and you can choose to withdraw from the study at any moment. There are no penalties for withdrawal. Also, you do not have to answer any question that makes you feel uncomfortable.

Your responses will be anonymous; your name will not be connected to any of your responses. This study is examining the judgments of groups, and is not designed to evaluate the performance of a particular individual. If the results of this study are published, your identity will be excluded.
