You are invited to participate. The researcher is a doctoral learner at Capella University. This form will discuss details of the study, allowing you to decide to participate, or not.  If you do not understand this form, ask the researcher. The purpose is to determine the relationship between peer mentoring support and novice nurse’s intent to stay in the field. About 120 participants will be in the study. You are invited to be in the study because you are: 18 years or older, you have been working or did work as an R N for 12 to 18 months continuously, and you had at least one peer mentoring support experience during your first 12-18 months at work (peer support that affords new nurses in transition psychological support e.g., trust, listening, and showing concern).

If you do not meet this description, you are not qualified for the study.  The researcher will not receive funds, and there is no cost to you. Your participation will last about 20 minutes. If you agree to be in this study, you will give data about yourself (e.g., gender, ethnicity, educational level, age -group), and complete a survey about the relationship between peer mentoring support and novice nurses intent to stay in the field. Being in this study will not benefit you, but data from this study may help others in the future. 

No study is completely risk-free. We don’t anticipate you will be harmed or distressed during this study. You may leave the study at any time if you are uncomfortable. Be aware there is a small chance responses could be viewed by unauthorized parties since your data is being entered and stored on a web server.  You will not receive anything for this study, and participation is voluntary. If you decide not to be in the study inform the researcher. 

Any data provided in this study that could identify you will be kept confidential. You are assured no classifying data will be used or collected at any time, and all outcomes will remain unidentifiable. Collection of data will be via electronic media, and only the researcher will have access. Your data will be in a locked file cabinet secured for a minimum of seven years unless needed for further use. Only the researcher, supervisor, and dissertation committee will have access to the data. Capella University’s IRB, Research Compliance Committee, or its designees may review your research records. If you leave the study, the researcher may still be able to use your data. 

You can ask questions about the study and call the researcher at any time. If you have any concerns, call the phone number listed on this form for questions (if any), or if you get hurt or sick during the study. Capella University’s Institutional Review Board (IRB) will protect the rights and welfare of human research participants.  Please contact us at 1-888-227-3552, ext. 6313, for any of these reasons: You wish to discuss concerns or suggestions to improve participant experience. You do not feel comfortable talking with the researcher. You do not need to give your name, but we may need to look at the data you provided, to follow up if you report a concern.
